⭐ Core Definition: Necrosis

Necrosis (from Ancient Greek νέκρωσις (nékrōsis) 'death') is a form of cell injury which results in the premature death of cells in living tissue by autolysis. The term "necrosis" came about in the mid-19th century and is commonly attributed to German pathologist Rudolf Virchow, who is often regarded as one of the founders of modern pathology. Necrosis is caused by factors external to the cell or tissue, such as infection, or trauma which result in the unregulated digestion of cell components. In contrast, apoptosis is a naturally occurring programmed and targeted cause of cellular death. While apoptosis often provides beneficial effects to the organism, necrosis is almost always detrimental and can be fatal.

Cellular death due to necrosis does not follow the apoptotic signal transduction pathway, but rather various receptors are activated and result in the loss of cell membrane integrity and an uncontrolled release of products of cell death into the extracellular space. This initiates an inflammatory response in the surrounding tissue, which attracts leukocytes and nearby phagocytes which eliminate the dead cells by phagocytosis. However, microbial damaging substances released by leukocytes would create collateral damage to surrounding tissues. This excess collateral damage inhibits the healing process. Thus, untreated necrosis results in a build-up of decomposing dead tissue and cell debris at or near the site of the cell death. A classic example is gangrene. For this reason, it is often necessary to remove necrotic tissue surgically, a procedure known as debridement.

Plague is an infectious disease caused by the bacterium Yersinia pestis. Symptoms include fever, weakness, headache and black lips. Usually this begins one to seven days after exposure. There are three forms of plague, each affecting a different part of the body and causing associated symptoms. Pneumonic plague infects the lungs, causing shortness of breath, coughing and chest pain; bubonic plague affects the lymph nodes, making them swell; and septicemic plague infects the blood and can cause tissues to turn black and die.

The bubonic and septicemic forms are generally spread by flea bites or handling an infected animal, whereas pneumonic plague is generally spread between people through the air via infectious droplets. Diagnosis is typically made by finding the bacterium in fluid from a lymph node, blood or sputum.

The remains of a former organism normally begin to decompose shortly after death. Venom or zootoxin is a type of toxin produced by an animal that is actively delivered through a wound by means of a bite, sting, or similar action. The toxin is delivered through a specially evolved venom apparatus, such as fangs or a stinger, in a process called envenomation. Venom is often distinguished from poison, which is a toxin that is passively delivered by being ingested, inhaled, or absorbed through the skin, and toxungen, which is actively transferred to the external surface of another animal via a physical delivery mechanism.

Venom has evolved in terrestrial and marine environments and in a wide variety of animals: both predators and prey, and both vertebrates and invertebrates. Rinderpest (also cattle plague or steppe murrain) was an infectious viral disease of cattle, domestic water buffalo, and many other species of even-toed ungulates, including gaurs, buffaloes, large antelope, deer, giraffes, wildebeests, and warthogs. The disease was characterized by fever, oral erosions, diarrhea, lymphoid necrosis, and high mortality. Death rates during outbreaks were usually extremely high, approaching 100% in immunologically naïve populations. Rinderpest was mainly transmitted by direct contact and by drinking contaminated water, although it could also be transmitted by air.

Rinderpest is believed to have originated in Asia, and to have spread by transport of cattle. The term Rinderpest (German: [ˈʁɪndɐˌpɛst] ) is a German word meaning 'cattle plague'. The rinderpest virus (RPV) is closely related to the measles and canine distemper viruses. The measles virus may have emerged from rinderpest as a zoonotic disease around 600 BC, a period that coincides with the rise of large human settlements. After a global eradication campaign that began in the mid-20th century, the last confirmed case of rinderpest was diagnosed in 2001. In 2010, the United Nations Food and Agriculture Organization (FAO) announced that field activities in the decades-long, worldwide campaign to eradicate the disease were ending, paving the way for a formal declaration in June 2011 of the global eradication of rinderpest. This makes it only the second disease in history to be fully wiped out, following smallpox.

The plague of Justinian or Justinianic plague (AD 541–549) was an epidemic of plague that afflicted the entire Mediterranean Basin, Europe, and the Near East, especially the Sasanian Empire and the Byzantine Empire. The plague is named for the Byzantine Emperor Justinian I (r. 527–565) who, according to his court historian Procopius, contracted the disease and recovered in 542, at the height of the epidemic which killed about a fifth of the population in the imperial capital Constantinople. The contagion arrived in Roman Egypt in 541, spread around the Mediterranean Sea until 544, and persisted in Northern Europe and the Arabian Peninsula until 549. By 543, the plague had spread to every corner of Justinian's empire.

The plague's severity and impact remain debated. Some scholars assert that as the first episode of the first plague pandemic, it had profound economic, social, and political effects across Europe and the Near East and cultural and religious impact on Eastern Roman society. However, scholars such as Mordechai and Eisenberg have argued that the Plague of Justinian was overstated by primary sources and while would have been incredibly impactful on a personal level, did not have a severe or even a long lasting impact on the population of the Mediterranean in the Age of Justinian.

Necrosis in the context of Gangrene

Gangrene is a type of tissue death caused by a lack of blood supply. Symptoms may include a change in skin color to red or black, numbness, swelling, pain, skin breakdown, and coolness. The feet and hands are most commonly affected. If the gangrene is caused by an infectious agent, it may present with a fever or sepsis.

Apoptosis (from Ancient Greek: ἀπόπτωσις, romanizedapóptōsis, lit.'falling off') is a form of programmed cell death that occurs in multicellular organisms and in some eukaryotic, single-celled microorganisms such as yeast. Biochemical events lead to characteristic cell changes (morphology) and death. These changes include blebbing, cell shrinkage, nuclear fragmentation, chromatin condensation, DNA fragmentation, and mRNA decay. The average adult human loses 50 to 70 billion cells each day due to apoptosis. For the average human child between 8 and 14 years old, each day the approximate loss is 20 to 30 billion cells.

In contrast to necrosis, which is a form of traumatic cell death that results from acute cellular injury, apoptosis is a highly regulated and controlled process that confers advantages during an organism's life cycle. For example, the separation of fingers and toes in a developing human embryo occurs because cells between the digits undergo a form of apoptosis that is genetically determined. Unlike necrosis, apoptosis produces cell fragments called apoptotic bodies that phagocytes are able to engulf and remove before the contents of the cell can spill out onto surrounding cells and cause damage to them.
