He was primarily associated with war, death, and disease, and has been described as the "god of inflicted death". He reigned over Kur, the Mesopotamian underworld, depending on the myth either on behalf of his parents Enlil and Ninlil, or in later periods as a result of his marriage with the goddess Ereshkigal. Originally either Mammitum, a goddess possibly connected to frost, or Laṣ, sometimes assumed to be a minor medicine goddess, were regarded as his wife, though other traditions existed, too.

His primary cult center was Kutha, located in the north of historical Babylonia. His main temple bore the ceremonial name E-Meslam and he was also known by the name Meslamtaea, "he who comes out of Meslam". Initially he was only worshiped in the north, with a notable exception being Girsu during the reign of Gudea of Lagash, but starting with the Ur III period he became a major deity in the south too. He remained prominent in both Babylonia and Assyria in later periods, and in the Neo-Babylonian state pantheon he was regarded as the third most important god, after Marduk and Nabu.

The Hurrian foundation pegs, also known as the Urkish lions, are twin copper foundation pegs each in the shape of a lion that probably came from the ancient city of Urkesh (modern Tell Mozan) in Syria. The pegs were placed at the foundation of the temple of Nergal in the city of Urkesh as mentioned in the cuneiform inscriptions on them. The inscription on the two pegs and the associated stone tablet is the oldest known text in the Hurrian language. One of the lions is now housed, along with its limestone tablet, in the Musée du Louvre in Paris. The second lion is on display at the Metropolitan Museum of Art in New York.

Nergal in the context of Ancient Mesopotamian underworld

The ancient Mesopotamian underworld (known in Sumerian as Kur, Irkalla, Kukku, Arali, or Kigal, and in Akkadian as Erṣetu), was the lowermost part of the ancient near eastern cosmos, roughly parallel to the region known as Tartarus from early Greek cosmology. It was described as a dark, dreary cavern located deep below the ground, where inhabitants were believed to continue "a transpositional version of life on earth". The only food or drink was dry dust, but family members of the deceased would pour sacred mineral libations from the earth for them to drink. In the Sumerian underworld, it was initially believed that there was no final judgement of the deceased and the dead were neither punished nor rewarded for their deeds in life.

The ruler of the underworld was the goddess Ereshkigal, who lived in the palace Ganzir, sometimes used as a name for the underworld itself. Her husband was either Gugalanna, the "canal-inspector of Anu", or, especially in later stories, Nergal, the god of war. After the Akkadian Period (c. 2334–2154 BC), Nergal sometimes took over the role as ruler of the underworld. The seven gates of the underworld are guarded by a gatekeeper, who is named Neti in Sumerian. The god Namtar acts as Ereshkigal's sukkal, or divine attendant. The dying god Dumuzid spends half the year in the underworld, while, during the other half, his place is taken by his sister, the scribal goddess Geshtinanna, who records the names of the deceased. The underworld was also the abode of various demons, including the hideous child-devourer Lamashtu, the fearsome wind demon and protector god Pazuzu, and galla, who dragged mortals to the underworld.

Nergal in the context of Neriglissar

Neriglissar (Babylonian cuneiform:   Nergal-šar-uṣur or Nergal-šarra-uṣur, meaning "Nergal, protect the king") was the fourth king of the Neo-Babylonian Empire, ruling from his usurpation of the throne in 560 BC to his death in 556 BC. Though unrelated to previous Babylonian kings, possibly being of Aramean ancestry, Neriglissar was a prominent official and general in the reign of Nebuchadnezzar II (r.605–562 BC) and became even more influential through marrying one of Nebuchadnezzar's daughters, possibly Kashshaya.

Nebuchadnezzar was initially succeeded by his son, Amel-Marduk, but Amel-Marduk's reign only lasted for two years before Neriglissar usurped the Babylonian throne and put him to death. Through his marriage to Nebuchadnezzar's daughter, possibly significantly older than any of the old king's sons, Neriglissar might have represented a less legitimate but more wealthy and well-established faction of the royal family, even if he himself was not part of this family.

Nergal in the context of Enlil

Enlil, later known as Elil and Ellil, is an ancient Mesopotamian god associated with wind, air, earth, and storms. He is first attested as the chief deity of the Sumerian pantheon, but he was later worshipped by the Akkadians, Babylonians, Assyrians, and Hurrians. Enlil's primary center of worship was the Ekur temple in the city of Nippur, which was believed to have been built by Enlil himself and was regarded as the "mooring-rope" of heaven and earth. He is also sometimes referred to in Sumerian texts as Nunamnir. According to one Sumerian hymn, Enlil himself was so holy that not even the other gods could look upon him. Enlil rose to prominence during the twenty-fourth century BC with the rise of Nippur. His cult fell into decline after Nippur was sacked by the Elamites in 1230 BC and he was eventually supplanted as the chief god of the Mesopotamian pantheon by the Babylonian national god Marduk.

Enlil plays a vital role in the ancient near eastern cosmology; he separates An (heaven) from Ki (earth), thus making the world habitable for humans. In the Sumerian flood myth Eridu Genesis, Enlil rewards Ziusudra with immortality for having survived the flood and, in the Babylonian flood myth, Enlil is the cause of the flood himself, having sent the flood to exterminate the human race, who made too much noise and prevented him from sleeping; the cuneiform tablets of Atra-Hasis report on this connections in a comparatively well-preserved state. The myth of Enlil and Ninlil is about Enlil's serial seduction of the goddess Ninlil in various guises, resulting in the conception of the moon-god Nanna and the Underworld deities Nergal, Ninazu, and Enbilulu. Enlil was regarded as the inventor of the mattock and the patron of agriculture. Enlil also features prominently in several myths involving his son Ninurta, including Anzû and the Tablet of Destinies and Lugale.

Nergal in the context of Ninlil

Ninlil (𒀭𒎏𒆤 NIN.LÍL; meaning uncertain) was a Mesopotamian goddess regarded as the wife of Enlil. She shared many of his functions, especially the responsibility for declaring destinies, and like him was regarded as a senior deity and head of the pantheon. She is also well attested as the mother of his children, such as the underworld god Nergal, the moon god Nanna or the warrior god Ninurta. She was chiefly worshiped in Nippur and nearby Tummal alongside Enlil, and multiple temples and shrines dedicated to her are attested in textual sources from these cities. In the first millennium BCE she was also introduced to Ḫursaĝkalamma near Kish, where she was worshiped alongside the goddess Bizilla, who was likely her sukkal (attendant deity).

At an early date Ninlil was identified with the goddess Sud from Shuruppak, like her associated with Enlil, and eventually fully absorbed her. In the myth Enlil and Sud, Ninlil is the name Sud received after marrying Enlil. Nisaba, the goddess of writing, and her husband Haya are described as her parents. While Ninlil's mother bears a different name, Nunbaršegunu, in the myth Enlil and Ninlil, the god list An = Anum states that it was an alternate name of Nisaba. Syncretism with Sud also resulted in Ninlil acquiring some of her unique characteristics, such as an association with healing goddesses and with Sudaĝ, a name of the wife of the sun god Shamash. References to these connections can be found in various Mesopotamian texts, such as a hymn referring to Ninlil as a healing goddess or a myth apparently confusing her with Sudaĝ in the role of mother of Ishum.

Nergal in the context of Lugal-irra and Meslamta-ea

Lugal-irra (𒀭𒈗𒄊𒊏) and Meslamta-ea (𒀭𒈩𒇴𒋫𒌓𒁺𒀀) were a pair of Mesopotamian gods who typically appear together in cuneiform texts and were described as the "divine twins" (Maštabba). They were regarded as warrior gods and as protectors of doors, possibly due to their role as the gatekeepers of the underworld. In Mesopotamian astronomy they came to be associated with a pair of stars known as the "Great Twins", Alpha Geminorum and Beta Geminorum. They were both closely associated with Nergal, and could be either regarded as members of his court or equated with him. Their cult centers were Kisiga and Dūrum. While no major sanctuaries dedicated to them are attested elsewhere, they were nonetheless worshiped in multiple other cities.
