Spinal muscular atrophy (SMA) is a rare neuromuscular disorder that results in the loss of motor neurons and progressive muscle wasting. The common feature is the progressive weakness of voluntary muscles, with the arm, leg, and respiratory muscles being affected first. The thoracic diaphragm, or simply the diaphragm (/ˈdəfræm/; Ancient Greek: διάφραγμα, romanizeddiáphragma, lit.'partition'), is a sheet of internal skeletal muscle in humans and other mammals that extends across the bottom of the thoracic cavity. The diaphragm is the most important muscle of respiration, and separates the thoracic cavity, containing the heart and lungs, from the abdominal cavity: as the diaphragm contracts, the volume of the thoracic cavity increases, creating a negative pressure there, which draws air into the lungs. Its high oxygen consumption is noted by the many mitochondria and capillaries present; more than in any other skeletal muscle.

The term diaphragm in anatomy, created by Gerard of Cremona, can refer to other flat structures such as the urogenital diaphragm or pelvic diaphragm, but "the diaphragm" generally refers to the thoracic diaphragm. In humans, the diaphragm is slightly asymmetric—its right half is higher up (superior) to the left half, since the large liver rests beneath the right half of the diaphragm.

The respiratory system (also respiratory apparatus, ventilatory system) is a biological system consisting of specific organs and structures used for gas exchange in animals and plants.

In land animals, the respiratory surface is internalized as linings of the lungs. Gas exchange in the lungs occurs in millions of small air sacs. In mammals and reptiles, these are called alveoli, and in birds, they are known as atria. These microscopic air sacs have a rich blood supply, bringing the air into close contact with the blood. A system of airways, or hollow tubes, allow the air sacs to interface with the external environment; the largest of these is the trachea, which branches in the middle of the chest into the two main bronchi, which enter the lungs and branch into progressively narrower secondary and tertiary bronchi, which in turn branch into numerous smaller tubes known as the bronchioles in mammals and reptiles. In birds, the bronchioles are termed parabronchi. The bronchioles, or parabronchi, generally open into the microscopic alveoli (in mammals) and atria (in birds). Air has to be pumped from the environment into the alveoli or atria by the process of breathing which involves the muscles of respiration.

Respiratory diseases, or lung diseases, are pathological conditions affecting the organs and tissues that make gas exchange difficult in air-breathing animals. They include conditions of the respiratory tract including the trachea, bronchi, bronchioles, alveoli, pleurae, pleural cavity, the nerves and muscles of respiration. Respiratory diseases range from mild and self-limiting, such as the common cold, influenza, and pharyngitis to life-threatening diseases such as bacterial pneumonia, pulmonary embolism, tuberculosis, acute asthma, lung cancer, chronic obstructive pulmonary disease and severe acute respiratory syndromes, such as COVID-19. Respiratory diseases can be classified in many different ways, including by the organ or tissue involved, by the type and pattern of associated signs and symptoms, or by the cause of the disease.

The study of respiratory disease is known as pulmonology. A physician who specializes in respiratory disease is known as a pulmonologist, a chest medicine specialist, a respiratory medicine specialist, a respirologist or a thoracic medicine specialist.
