The Mantoux test (also called the Mendel–Mantoux test, tuberculin sensitivity test, or PPD test) is a method used to screen for tuberculosis (TB) infection. It has largely replaced older skin testing techniques such as the tine and Heaf tests. The test involves injecting a small amount of purified protein derivative (PPD) tuberculin just under the skin of the forearm. If performed correctly, the injection creates a small, pale bump called a wheal. The test site is examined a few days later for swelling or hardening of the skin, an immune response that would be expected if the person had been exposed to tuberculosis. However, additional tests are usually required to confirm active infection.

Tuberculosis (TB) (RP:/tjˈbɜːrkjˌlsɪs/ tew-BER-kew-loh-sis, also /ˌtjbərkjˈlsɪs/ tew-bər-kew-LOH-sis), also known colloquially as the "white death", or historically as consumption, is a contagious disease usually caused by Mycobacterium tuberculosis (MTB) bacteria. Tuberculosis generally affects the lungs, but it can also affect other parts of the body. Most infections show no symptoms, in which case it is known as inactive or latent tuberculosis. A small proportion of latent infections progress to active disease that, if left untreated, can be fatal. Typical symptoms of active TB are chronic cough with blood-containing mucus, fever, night sweats, and weight loss. Infection of other organs can cause a wide range of symptoms.

Tuberculosis is spread from one person to the next through the air when people who have active TB in their lungs cough, spit, speak, or sneeze. People with latent TB do not spread the disease. A latent infection is more likely to become active in those with weakened immune systems. There are two principal tests for TB: interferon-gamma release assay (IGRA) of a blood sample, and the tuberculin skin test.

Mycobacterium tuberculosis (M. tb), also known as Koch's bacillus, is a species of pathogenic bacteria in the family Mycobacteriaceae and the causative agent of tuberculosis.

First discovered in 1882 by Robert Koch, M. tuberculosis has an unusual, waxy coating on its cell surface primarily due to the presence of mycolic acid. This coating makes the cells impervious to Gram staining, and as a result, M. tuberculosis can appear weakly Gram-positive. Acid-fast stains such as Ziehl–Neelsen, or fluorescent stains such as auramine are used instead to identify M. tuberculosis with a microscope. The physiology of M. tuberculosis is highly aerobic and requires high levels of oxygen. Primarily a pathogen of the mammalian respiratory system, it infects the lungs. The most frequently used diagnostic methods for tuberculosis are the tuberculin skin test, acid-fast stain, culture, and polymerase chain reaction.

Mantoux test in the context of Tine test

The tine test is a multiple-puncture tuberculin skin test used to aid in the medical diagnosis of tuberculosis (TB). The tine test is similar to the Heaf test, although the Mantoux test is typically used instead. There are various forms of the tine tests which usually fall into two categories: the old tine test (OT) and the purified protein derivative (PPD) tine test. Common brand names of the test include Aplisol, Aplitest, Tuberculin PPD TINE TEST, and Tubersol.

Mantoux test in the context of Heaf test

The Heaf test, a diagnostic skin test, was long performed to determine whether or not children had been exposed to tuberculosis infection. The test was named after F. R. G. Heaf. Also known as the Sterneedle test, it was administered by a Heaf gun (trademarked "Sterneedle"), a spring-loaded instrument with six needles arranged in a circular formation which was inserted in the wrist or shoulder.

The Heaf test was discontinued in 2005 because the manufacturer deemed its production to be financially unsustainable after manufacturers could not be found for tuberculin or Heaf guns. Until 2005, the test was used in the United Kingdom to determine if the BCG vaccine was needed; the Mantoux test is now used instead. The Heaf test was preferred in the UK, because it was thought to be easier to interpret, with less variability between observers, and less training was required to administer and read the test.
