Gout (/ɡt/ GOWT) is a form of inflammatory arthritis characterized by recurrent attacks of pain in a red, tender, hot, and swollen joint, caused by the deposition of needle-shaped crystals of the monosodium salt of uric acid. Pain typically comes on rapidly, reaching maximal intensity in less than 12 hours. In about half of cases, the joint at the base of the big toe is affected (Podagra). It may also result in tophi, kidney stones, or kidney damage.

Gout is due to persistently elevated levels of uric acid (urate) in the blood (hyperuricemia). This occurs from a combination of diet, other health problems, and genetic factors. At high levels, uric acid crystallizes and the crystals deposit in joints, tendons, and surrounding tissues, resulting in an attack of gout. Gout occurs more commonly in those who regularly drink beer or sugar-sweetened beverages; eat foods that are high in purines such as liver, shellfish, or anchovies; or are overweight. Diagnosis of gout may be confirmed by the presence of crystals in the joint fluid or in a deposit outside the joint. Blood uric acid levels may be normal during an attack.

Diseases of affluence, previously called diseases of rich people, is a term sometimes given to selected diseases and other health conditions which are commonly thought to be a result of increasing wealth in a society. Also referred to as the "Western disease" paradigm, these diseases are in contrast to "diseases of poverty", which largely result from and contribute to human impoverishment. These diseases of affluence have vastly increased in prevalence since the end of World War II.

Uric acid is a heterocyclic compound of carbon, nitrogen, oxygen, and hydrogen with the formula C5H4N4O3. It forms ions and salts known as urates and acid urates, such as ammonium acid urate. Uric acid is a product of the metabolic breakdown of purine nucleotides, and it is a normal component of urine. Gout in the context of Lesch-Nyhan Syndrome

Lesch–Nyhan syndrome (LNS) is a rare inherited disorder caused by a deficiency of the enzyme hypoxanthine-guanine phosphoribosyltransferase (HGPRT). This deficiency occurs due to mutations in the HPRT1 gene located on the X chromosome. LNS affects about 1 in 380,000 live births. The disorder was first recognized and clinically characterized by American medical student Michael Lesch and his mentor, pediatrician William Nyhan, at Johns Hopkins.

The HGPRT deficiency causes a build-up of uric acid in all body fluids. The combination of increased synthesis and decreased utilization of purines leads to high levels of uric acid production. This results in both high levels of uric acid in the blood and urine, associated with severe gout and kidney problems. Neurological signs include poor muscle control and moderate intellectual disability. These complications usually appear in the first year of life. Beginning in the second year of life, a particularly striking feature of LNS is self-mutilating behaviors, characterized by lip and finger biting. Neurological symptoms include facial grimacing, involuntary writhing, and repetitive movements of the arms and legs similar to those seen in Huntington's disease. The cause of the neurological abnormalities remains unknown. Because a lack of HGPRT causes the body to poorly utilize vitamin B12, some males may develop megaloblastic anemia.

Arthrocentesis, or joint aspiration, is the clinical procedure performed to diagnose and, in some cases, treat musculoskeletal conditions. The procedure entails using a syringe to collect synovial fluid from or inject medication into the joint capsule. Laboratory analysis of synovial fluid can further help characterize the diseased joint and distinguish between gout, arthritis, and synovial infections such as septic arthritis.

Gout in the context of Colchicine

Colchicine is a medication used to prevent and treat gout, to treat familial Mediterranean fever and Behçet's disease, and to reduce the risk of myocardial infarction. The American College of Rheumatology recommends colchicine, nonsteroidal anti-inflammatory drugs (NSAIDs) or steroids in the treatment of gout. Other uses for colchicine include the management of pericarditis.

Colchicine is taken by mouth. The injectable route of administration for colchicine can be toxic. In 2008, the US Food and Drug Administration removed all injectable colchicine from the US market.

Michel de Nostredame (December 1503 – July 1566), usually Latinised as Nostradamus, was a French astrologer, apothecary, physician, and reputed seer, who is best known for his book Les Prophéties (published in 1555), a collection of 942 poetic quatrains allegedly predicting future events.

Nostradamus's father's family had originally been Jewish, but had converted to Catholic Christianity a generation before Nostradamus was born. He studied at the University of Avignon, but was forced to leave after just over a year when the university closed due to an outbreak of the plague. He worked as an apothecary for several years before entering the University of Montpellier, hoping to earn a doctorate, but was almost immediately expelled after his work as an apothecary (a manual trade forbidden by university statutes) was discovered. He first married in 1531, but his wife and two children died in 1534 during another plague outbreak. He worked against the plague alongside other doctors before remarrying to Anne Ponsarde, with whom he had six children. He wrote an almanac for 1550 and, as a result of its success, continued writing them for future years as he began working as an astrologer for various wealthy patrons. Catherine de' Medici became one of his foremost supporters. His Les Prophéties, published in 1555, relied heavily on historical and literary precedent, and initially received mixed reception. He suffered from severe gout toward the end of his life, which eventually developed into edema. He died on 1 or 2 July 1566. Many popular authors have retold apocryphal legends about his life.
