The codex (pl.: codices /ˈkdɪsz/) was the historical ancestor format of the modern book. Technically, the vast majority of modern books use the codex format of a stack of pages bound at one edge, along the side of the text. However, the term codex is now reserved for older manuscript books, which mostly used sheets of vellum, parchment, or papyrus, rather than paper.

By convention, the term is also used for any Aztec codex (although the earlier examples do not actually use the codex format), Maya codices and other pre-Columbian manuscripts. In contrast, while animal skin was never used to write books in eastern and southern Asia, it became a mainstay for prestige manuscripts in the Middle East, Europe, and the Americas. Similarly, papyrus and even paper were used in different regions at various times, reflecting local resource availability and cultural needs. Across regions like China, the Middle East, Europe, and South Asia, diverse methods of book production evolved. The Middle Ages saw the rise of illuminated manuscripts, intricately blending text and imagery, particularly during the Mughal era in South Asia under the patronage of rulers like Akbar and Shah Jahan. In Muslim tradition the Quran is the final revelation from God, Islam's divine text, believed to be delivered to the Islamic prophet Muhammad through the angel Jibril (Gabriel). Muhammad's revelations were said to have been recorded orally and in writing, through Muhammad and his followers up until his death in 632 CE. These revelations were then compiled by first caliph Abu Bakr and codified during the reign of the third caliph Uthman (r. 644–656 CE) so that the standard codex edition of the Quran or Muṣḥaf was completed around 650 CE, according to Muslim scholars. This has been critiqued by some western scholarship, suggesting the Quran was canonized at a later date, based on the dating of classical Islamic narratives, i.e. hadiths, which were written 150–200 years after the death of Muhammad, and partly because of the textual variations present in the Sana'a manuscript. Muslim scholars who oppose the views of the Western revisionist theories regarding the historical origins of the Quran have described their theses as "untenable".

More than 60 fragments including more than 2000 folios (4000 pages) are so far known as the textual witnesses (manuscripts) of the Qur'an before 800 CE (within 168 years after the death of Muhammad), according to Corpus Coranicum. However, in 2015, experts from the University of Birmingham discovered a single folio later called the Birmingham Quran manuscript, which is possibly the oldest manuscript of the Quran in the world. Radiocarbon analysis to determine the age of the manuscript revealed that this manuscript could be traced back to some time between 568 and 645 AD. If meant for liturgic purposes, it takes the form of a Torah scroll (Hebrew: ספר תורה Sefer Torah). If in bound book form, it is called Chumash, and is usually printed with the rabbinic commentaries (perushim).

In rabbinic literature, the word Torah denotes both the five books (תורה שבכתב, "Torah that is written") and the Oral Torah (תורה שבעל פה, "Torah that is spoken"). It has also been used, however, to designate the entire Hebrew Bible. The Oral Torah consists of interpretations and amplifications which according to rabbinic tradition have been handed down from generation to generation and are now embodied in the Talmud and Midrash. Rabbinic tradition's understanding is that all of the teachings found in the Torah (both written and oral) were given by God through the prophet Moses, some at Mount Sinai and others at the Tabernacle, and all the teachings were written down by Moses, which resulted in the Torah that exists today. According to the Midrash, the Torah was created prior to the creation of the world, and was used as the blueprint for Creation. Though hotly debated, the general trend in biblical scholarship is to recognize the final form of the Torah as a literary and ideological unity, based on earlier sources, largely complete by the Persian period, with possibly some later additions during the Hellenistic period.

The Voynich manuscript is an illustrated codex, hand-written in an unknown script referred to as Voynichese. The vellum on which it is written has been carbon-dated to the early 15th century (1404–1438). Stylistic analysis has indicated the manuscript may have been composed in Italy during the Italian Renaissance. The origins, authorship, and purpose of the manuscript are still debated, but currently scholars lack the translation(s) and context needed to either properly entertain or eliminate any of the possibilities. Hypotheses range from a script for a natural language or constructed language, an unreadable code, cypher, or other form of cryptography, or perhaps a hoax, reference work (i.e. folkloric index or compendium), glossolalia or work of fiction (e.g. science fantasy or mythopoeia, metafiction, speculative fiction).

The first confirmed owner was Georg Baresch, a 17th-century alchemist from Prague. The manuscript is named after Wilfrid Voynich, a Polish book dealer who purchased it in 1912. The manuscript consists of around 240 pages, but there is evidence that some of the pages are missing. The text is written from left to right, and some pages are foldable sheets of varying sizes. Most of the pages have fantastical illustrations and diagrams, some crudely coloured, with sections of the manuscript showing people, unidentified plants and astrological symbols. Since 1969, it has been held in Yale University's Beinecke Rare Book and Manuscript Library. In 2020, Yale University published the manuscript online in its entirety in their digital library.

A lost literary work (referred throughout this article just as a lost work) is a document, literary work, or piece of multimedia, produced of which no surviving copies are known to exist, meaning it can be known only through reference, or literary fragments. Recto is the "right" or "front" side and verso is the "left" or "back" side when text is written or printed on a leaf of paper (folium) in a bound item such as a codex, book, broadsheet, or pamphlet.

The trade of bookbinding includes the binding of blank books and printed books. Blank books, or stationery bindings, are books planned to be written in.
