Officially organized on October 1, 1812, the county is named after Ensign Sieur Jean Baptiste de Girardot, an official of the French colonial years. The "cape" in the county's name is named after a former promontory rock overlooking the Mississippi River; this feature was demolished during railroad construction. Cape Girardeau County is the hub of the Cape Girardeau–Jackson metropolitan area. Its largest city is Cape Girardeau.

There are 114 counties and one independent city in the U.S. State of Missouri. Following the Louisiana Purchase and the admittance of Louisiana into the United States in 1812, five counties were formed out of the Missouri Territory at the first general assembly: Cape Girardeau, New Madrid, Saint Charles, Saint Louis, and Ste. Genevieve. Most subsequent counties were apportioned from these five original counties. Six more counties were added through the 1836 Platte Purchase, the acquired lands of which formed the northwest tip of the state and consisted of Andrew, Atchison, Buchanan, Holt, Nodaway, and Platte counties.

In Missouri, the county level of government comes between those of the city and the state. Its primary responsibilities include maintaining roads, providing security, prosecuting criminals, and collecting taxes. Elected officials at this level include a sheriff, prosecuting attorney, and assessor.

Cape Girardeau (/ɪˈrɑːrd/ jirr-AR-doh, French: Cap-Girardeau [kap ʒiʁaʁdo] ; colloquially referred to as "Cape") is a city in Cape Girardeau and Scott counties in the U.S. state of Missouri. The population was 39,540 at the 2020 census, making it the 17th-largest city in the state. It is a principal city of the Cape Girardeau–Jackson metropolitan area with 97,517 residents. Cape Girardeau is the economic center of southeastern Missouri and the home of Southeast Missouri State University. It is located approximately 100 miles (160 km) southeast of St. Louis and 150 miles (240 km) north of Memphis, Tennessee.
