Split-brain or callosal syndrome is a type of disconnection syndrome when the corpus callosum connecting the two hemispheres of the brain is severed to some degree. It is an association of symptoms produced by disruption of, or interference with, the connection between the hemispheres of the brain. The surgical operation to produce this condition (corpus callosotomy) involves transection of the corpus callosum, and is usually a last resort to treat refractory epilepsy. Initially, partial callosotomies are performed; if this operation does not succeed, a complete callosotomy is performed to mitigate the risk of accidental physical injury by reducing the severity and violence of epileptic seizures. Before using callosotomies, epilepsy is instead treated through pharmaceutical means. After surgery, neuropsychological assessments are often performed.

After the right and left brain are separated, each hemisphere will have its own separate perception, concepts, and impulses to act. Having two "brains" in one body can create some interesting dilemmas. There was a case in which, when one split-brain patient would dress himself, sometimes he pulled his pants up with one hand (the side of his brain that wanted to get dressed) and down with the other (the side that did not). He was also reported to have grabbed his wife with his left hand and shook her violently, at which point his right hand came to her aid and grabbed the aggressive left hand (a phenomenon sometimes occurring, known as alien hand syndrome). However, such conflicts are very rare. If a conflict arises, one hemisphere usually overrides the other.

Split-brain in the context of Lateralization of brain function

The lateralization of brain function (or hemispheric dominance/ lateralization) is the tendency for some neural functions or cognitive processes to be specialized to one side of the brain or the other. The median longitudinal fissure separates the human brain into two distinct cerebral hemispheres connected by the corpus callosum. Both hemispheres exhibit brain asymmetries in both structure and neuronal network composition associated with specialized function.

Lateralization of brain structures has been studied using both healthy and split-brain patients. However, there are numerous counterexamples to each generalization and each human's brain develops differently, leading to unique lateralization in individuals. This is different from specialization, as lateralization refers only to the function of one structure divided between two hemispheres. Specialization is much easier to observe as a trend, since it has a stronger anthropological history.

Split-brain in the context of Disconnection syndrome

Disconnection syndrome is a general term for a collection of neurological symptoms caused – via lesions to associational or commissural nerve fibres – by damage to the white matter axons of communication pathways in the cerebrum (not to be confused with the cerebellum), independent of any lesions to the cortex. The behavioral effects of such disconnections are relatively predictable in adults. Disconnection syndromes usually reflect circumstances where regions A and B still have their functional specializations except in domains that depend on the interconnections between the two regions.

Callosal syndrome, or split-brain, is an example of a disconnection syndrome from damage to the corpus callosum between the two hemispheres of the brain. Disconnection syndrome can also lead to aphasia, left-sided apraxia, and tactile aphasia, among other symptoms. Other types of disconnection syndrome include conduction aphasia (lesion of the association tract connecting Broca’s area and Wernicke’s), agnosia, apraxia, pure alexia, etc.
