The National Transportation Safety Board (NTSB) is an independent U.S. government investigative agency responsible for civil transportation accident investigation. In this role, the NTSB investigates and reports on aviation accidents and incidents, certain types of highway crashes, ship and marine accidents, pipeline incidents, bridge failures, and railroad accidents. The NTSB is also in charge of investigating cases of hazardous materials releases that occur during transportation. The agency is based in Washington, D.C. It has three regional offices, located in Anchorage, Alaska; Aurora, Colorado; and Federal Way, Washington. The agency also operated a national training center at its Ashburn facility.

National Transportation Safety Board in the context of Civil Aeronautics Administration (United States)

The Air Commerce Act of 1926 created an Aeronautic Branch of the United States Department of Commerce. Its functions included testing and licensing of pilots, certification of aircraft and investigation of accidents.

In 1934, the Aeronautics Branch was renamed the Bureau of Air Commerce, to reflect the growing importance of commercial flying. It was subsequently divided into two authorities: the Civil Aeronautics Administration (CAA), concerned with air traffic control, and the Civil Aeronautics Board (CAB), concerned with safety regulations and accident investigation. Under the Federal Aviation Act of 1958, the CAA's powers were transferred to a new independent body, the Federal Aviation Administration (FAA). In the same year, the National Aeronautics and Space Administration (NASA) was created after the Soviet Union’s launch of the first artificial satellite.The accident investigation powers of the CAB were transferred to the new National Transportation Safety Board in 1967, at the same time that the United States Department of Transportation was created.

The I-35W Mississippi River bridge (officially known as Bridge 9340) was an eight-lane, steel truss arch bridge that carried Interstate 35W across the Mississippi River one-half mile (875 m) downstream from the Saint Anthony Falls in Minneapolis, Minnesota, United States. The bridge opened in 1967, and was Minnesota's third busiest, carrying 140,000 vehicles daily. After 39 years in service, it experienced a catastrophic failure during the evening rush hour on August 1, 2007, killing 13 people and injuring 145. The National Transportation Safety Board (NTSB) cited a design flaw as the likely cause of the collapse, noting that an excessively thin gusset plate ripped along a line of rivets. The amount of weight on the bridge at the time of failure was also cited by the NTSB as a contributing factor.

Help came immediately from mutual aid in the seven-county Minneapolis–Saint Paul metropolitan area and emergency response personnel, charities, and volunteers. Within a few days of the collapse, the Minnesota Department of Transportation (MnDOT) planned its replacement with the I-35W Saint Anthony Falls Bridge. The construction of the replacement bridge was completed quickly, and the new bridge officially opened on September 18, 2008.

National Transportation Safety Board in the context of United Airlines Flight 389

United Air Lines Flight 389 was a scheduled flight from LaGuardia Airport, New York City, New York, to O'Hare International Airport, Chicago, Illinois. On August 16, 1965, at approximately 21:21 EST, the Boeing 727 crashed into Lake Michigan 20 miles (17 nmi; 32 km) east of Fort Sheridan, near Lake Forest, while descending from 35,000 feet (11,000 m) mean sea level (MSL). There was no indication of any unusual problem prior to impact. All 30 persons aboard, including six crew members and 24 passengers, were killed.

A definitive cause was not determined by National Transportation Safety Board (NTSB) investigators. However, it was believed that the crash was most likely the result of the pilots misreading their three-pointer (3p) altimeters by 10,000 feet.
