Dengue virus (DENV) is the cause of dengue fever. It is a mosquito-borne, single positive-stranded RNA virus of the family Flaviviridae; genus Orthoflavivirus. Four serotypes of the virus have been found, and a reported fifth has yet to be confirmed, all of which can cause the full spectrum of disease. Nevertheless, the mainstream scientific community's understanding of dengue virus may be simplistic as, rather than distinct antigenic groups, a continuum appears to exist. This same study identified 47 strains of dengue virus. Additionally, coinfection with and lack of rapid tests for Zika virus and chikungunya complicate matters in real-world infections.

Dengue virus has increased dramatically within the last 20 years, becoming one of the worst mosquito-borne human pathogens that tropical countries have to deal with. Estimates published in 2013 indicate that as many as 390 million infections occur each year, and many dengue infections are increasingly understood to be asymptomatic or subclinical.

Dengue virus in the context of Dengue fever

Dengue fever is a mosquito-borne disease caused by dengue virus, prevalent in tropical and subtropical areas. Most cases of dengue fever are either asymptomatic or manifest mild symptoms. Symptoms typically begin 3 to 14 days after infection. They may include a high fever, headache, vomiting, muscle and joint pains, and a characteristic skin itching and skin rash. Recovery generally takes two to seven days. In a small proportion of cases, the disease develops into severe dengue (previously known as dengue hemorrhagic fever or dengue shock syndrome) with bleeding, low levels of blood platelets, blood plasma leakage, and dangerously low blood pressure.

Dengue is spread by several species of female mosquitoes of the Aedes genus, principally Aedes aegypti. The virus has four confirmed serotypes; infection with one type usually gives lifelong immunity to that type, but only short-term immunity to the others. Subsequent infection with a different type increases the risk of severe complications, so-called antibody-dependent enhancement. The symptoms may resemble those of malaria, influenza, or Zika. Blood tests to confirm the diagnosis including detecting viral RNA, or specific antibodies.

An RNA virus is a virus characterized by a ribonucleic acid (RNA) based genome. The genome can be single-stranded RNA (ssRNA) or double-stranded (dsRNA). Notable human diseases caused by RNA viruses include influenza, SARS, MERS, COVID-19, Dengue virus, hepatitis C, hepatitis E, West Nile fever, Ebola virus disease, rabies, polio, mumps, and measles.

All RNA viruses use a homologous RNA-dependent polymerase for replication and are categorized by the International Committee on Taxonomy of Viruses (ICTV) into the realm Riboviria. This includes viruses belonging to Group III, Group IV, Group V, and Group VI of the Baltimore classification system. Group VI comprises the retroviruses, which have RNA genetic material but use DNA intermediates in their life cycle. Riboviria does not include viroids and satellite nucleic acids: Deltavirus, Avsunviroidae, and Pospiviroidae are taxa that were mistakenly included in 2019, but this was corrected in 2020.

Dengue virus in the context of Orthoflavivirus

Orthoflavivirus (Flavivirus prior to 2023; common name orthoflavivirus, orthoflaviviral or orthoflaviviruses) is a genus of positive-strand RNA viruses in the family Flaviviridae. The genus includes the West Nile virus, dengue virus, tick-borne encephalitis virus, yellow fever virus, Zika virus and several other viruses which may cause encephalitis, as well as insect-specific flaviviruses (ISFs) such as cell fusing agent virus (CFAV), Palm Creek virus (PCV), and Parramatta River virus (PaRV). While dual-host flaviviruses can infect vertebrates as well as arthropods, insect-specific flaviviruses are restricted to their competent arthropods. The means by which flaviviruses establish persistent infection in their competent vectors and cause disease in humans depends upon several virus-host interactions, including the intricate interplay between flavivirus-encoded immune antagonists and the host antiviral innate immune effector molecules.

Orthoflaviviruses are named for the yellow fever virus; the word flavus means 'yellow' in Latin, and yellow fever in turn is named from its propensity to cause yellow jaundice in victims.

Dengue virus in the context of Antibody-dependent enhancement

Antibody-dependent enhancement (ADE), sometimes less precisely called immune enhancement or disease enhancement, is a phenomenon in which binding of a virus to suboptimal antibodies enhances its entry into host cells, followed by its replication. The suboptimal antibodies can result from natural infection or from vaccination. ADE may cause enhanced respiratory disease, but is not limited to respiratory disease. It has been observed in HIV, RSV, and Dengue virus and is monitored for in vaccine development.
