The cervix (pl.: cervices) or uterine cervix (Latin: cervix uteri) is a dynamic fibromuscular sexual organ of the female reproductive system that connects the vagina with the uterine cavity. The human female cervix has been documented anatomically since at least the time of Hippocrates, over 2,000 years ago. The cervix is approximately 4 cm (1.6 in) long with a diameter of approximately 3 cm (1.2 in) and tends to be described as a cylindrical shape, although the front and back walls of the cervix are contiguous. The size of the cervix changes throughout a female's life cycle. For example, females in the fertile years of their reproductive cycle tend to have larger cervixes than postmenopausal females; likewise, females who have produced offspring have a larger cervix than those who have not.

In relation to the vagina, the part of the cervix that opens into the uterus is called the internal os while the opening of the cervix into the vagina is called the external os. Between those extremes is the conduit commonly called the cervical canal. The lower part of the cervix, known as the vaginal portion of the cervix (or ectocervix), bulges into the top of the vagina. The endocervix borders the uterus. The cervical conduit has at least two types of epithelium (lining): the endocervical lining is glandular epithelia that lines the endocervix with a single layer of column-shaped cells; while the ectocervical part of the conduit contains squamous epithelium. Squamous epithelia line the conduit with multiple layers of cells topped with flat cells. These two linings converge at the squamocolumnar junction (SCJ). This junction changes location dynamically throughout a female's life. The cervix is the organ that allows epithelia to flow from a female's uterus and out through her vagina at menstruation. Menstruation releases epithelia from a female’s uterus with every period of her fertile years, unless pregnancy occurs.

Cervix in the context of Abortion

Abortion is the termination of a pregnancy by removal or expulsion of an embryo or fetus. The unmodified word abortion generally refers to induced abortion, or deliberate actions to end a pregnancy. Abortion occurring without intervention is known as spontaneous abortion or "miscarriage", and occurs in roughly 30–40% of all pregnancies. Common reasons for inducing an abortion are birth-timing and limiting family size. Other reasons include maternal health, an inability to afford a child, domestic violence, lack of support, feelings of being too young, wishing to complete an education or advance a career, and not being able, or willing, to raise a child conceived as a result of rape or incest.

When done legally in industrialized societies, induced abortion is one of the safest procedures in medicine. Modern methods use medication or surgery for abortions. The drug mifepristone (aka RU-486) in combination with prostaglandin appears to be as safe and effective as surgery during the first and second trimesters of pregnancy. Self-managed medication abortion is highly effective and safe throughout the first trimester. The most common surgical technique involves dilating the cervix and using a suction device. Birth control, such as the pill or intrauterine devices, can be used immediately following an abortion. When performed legally and safely on a woman who desires it, an induced abortion does not increase the risk of long-term mental or physical problems. In contrast, unsafe abortions performed by unskilled individuals, with hazardous equipment, or in unsanitary facilities cause between 22,000 and 44,000 deaths and 6.9 million hospital admissions each year — responsible for between 5% and 13% of maternal deaths, especially in low-income countries. The World Health Organization states that "access to legal, safe and comprehensive abortion care, including post-abortion care, is essential for the attainment of the highest possible level of sexual and reproductive health". Public health data show that making safe abortion legal and accessible reduces maternal deaths.

Cervix in the context of Neck

The neck is the part of the body in many vertebrates that connects the head to the torso. It supports the weight of the head and protects the nerves that transmit sensory and motor information between the brain and the rest of the body. Additionally, the neck is highly flexible, allowing the head to turn and move in all directions. Anatomically, the human neck is divided into four compartments: vertebral, visceral, and two vascular compartments. Within these compartments, the neck houses the cervical vertebrae, the cervical portion of the spinal cord, upper parts of the respiratory and digestive tracts, endocrine glands, nerves, arteries⁣⁣ and veins. The muscles of the neck, which are separate from the compartments, form the boundaries of the neck triangles.

In anatomy, the neck is also referred to as the cervix or collum. However, when the term cervix is used alone, it often refers to the uterine cervix, the neck of the ⁣⁣uterus⁣⁣. Therefore, the adjective cervical can refer either to the neck (as in cervical vertebrae or cervical lymph nodes) or to the uterine cervix (as in cervical cap or cervical cancer).

Cervix in the context of Spontaneous abortion

Miscarriage, also known in medical terms as a spontaneous abortion, is an end to pregnancy resulting in the loss and expulsion of an embryo or fetus from the womb before it can survive independently. Miscarriage before 6 weeks of gestation is defined as biochemical loss by ESHRE. Once ultrasound or histological evidence shows that a pregnancy has existed, the term used is clinical miscarriage, which can be "early" (before 12 weeks) or "late" (between 12 and 21 weeks). Spontaneous fetal termination after 20 weeks of gestation is known as a stillbirth. The term miscarriage is sometimes used to refer to all forms of pregnancy loss and pregnancy with abortive outcomes before 20 weeks of gestation.

The most common symptom of a miscarriage is vaginal bleeding, with or without pain. Tissue and clot-like material may leave the uterus and pass through and out of the vagina. Risk factors for miscarriage include being an older parent, previous miscarriage, exposure to tobacco smoke, obesity, diabetes, thyroid problems, and drug or alcohol use. About 80% of miscarriages occur in the first 12 weeks of pregnancy (the first trimester). The underlying cause in about half of cases involves chromosomal abnormalities. Diagnosis of a miscarriage may involve checking to see if the cervix is open or sealed, testing blood levels of human chorionic gonadotropin (hCG), and an ultrasound. Other conditions that can produce similar symptoms include an ectopic pregnancy and implantation bleeding.

Cervix in the context of Mifepristone

Mifepristone, and also known by its developmental code name RU-486, is a drug typically used in combination with misoprostol to bring about a medical abortion during pregnancy. This combination is 97% effective during the first 63 days (9 weeks) of pregnancy, and is effective in the second trimester as well. It is also used on its own to treat Cushing's syndrome or in low doses as an emergency contraceptive.

The most common adverse effects include abdominal pain, feeling tired, and vaginal bleeding. Serious side effects may include heavy vaginal bleeding, bacterial infection, and, if pregnant, birth defects. When used, appropriate follow-up care needs to be available. Mifepristone is primarily an antiprogestogen. It works by blocking the effects of progesterone, making both the cervix and uterine vessels dilate and causing uterine contraction. Mifepristone also works, to a lesser extent, as an antiglucocorticoid and diminishes the effects of hypercortisolism.

Cervix in the context of Dilation and evacuation

Dilation and evacuation (D&E) or dilatation and evacuation (British English) is the dilation of the cervix and surgical evacuation of the uterus (potentially including the fetus, placenta and other tissue) after the first trimester of pregnancy. It is the most common method and procedure for abortions in the second trimester of pregnancy. The procedure can also be used to remove a miscarried fetus from the womb.

Vacuum or suction aspiration is a procedure that uses a vacuum source to remove an embryo or fetus through the cervix. The procedure is performed to induce abortion, as a treatment for incomplete spontaneous abortion (otherwise commonly known as miscarriage) or retained fetal and placental tissue, or to obtain a sample of uterine lining (endometrial biopsy). It is generally safe, and serious complications rarely occur.

Some sources may use the terms dilation and evacuation or "suction" dilation and curettage to refer to vacuum aspiration, although those terms are normally used to refer to distinctly different procedures.

The uterus (from Latin uterus, pl.: uteri or uteruses) or womb (/wm/) is the organ in the reproductive system of most female mammals, including humans, that accommodates the embryonic and fetal development of one or more fertilized eggs until birth. The uterus is a hormone-responsive sex organ that contains glands in its lining that secrete uterine milk for embryonic nourishment. (The term uterus is also applied to analogous structures in some non-mammalian animals.)

In humans, the lower end of the uterus is a narrow part known as the isthmus that connects to the cervix, the anterior gateway leading to the vagina. The upper end, the body of the uterus, is connected to the fallopian tubes at the uterine horns; the rounded part, the fundus, is above the openings to the fallopian tubes. The connection of the uterine cavity with a fallopian tube is called the uterotubal junction. The fertilized egg is carried to the uterus along the fallopian tube.
