Aphasia, also known as dysphasia, is an impairment in a person's ability to comprehend or formulate language because of dysfunction in specific brain regions. The major causes are stroke and head trauma; prevalence is hard to determine, but aphasia due to stroke is estimated to be 0.1–0.4% in developed countries. Aphasia can also be the result of brain tumors, epilepsy, autoimmune neurological diseases, brain infections, or neurodegenerative diseases (such as dementias).

To be diagnosed with aphasia, a person's language must be significantly impaired in one or more of the four aspects of communication. In the case of progressive aphasia, a noticeable decline in language abilities over a short period of time is required. The four aspects of communication include spoken language production, spoken language comprehension, written language production, and written language comprehension. Impairments in any of these aspects can impact functional communication.

Aphasia in the context of Broca's area

Broca's area, or the Broca area (/ˈbrkə/, also UK: /ˈbrɒkə/, US: /ˈbrkɑː/), is a region in the frontal lobe of the dominant hemisphere, usually the left, of the brain with functions linked to speech production.

Language processing has been linked to Broca's area since Pierre Paul Broca reported impairments in two patients. They had lost the ability to speak after injury to the posterior inferior frontal gyrus (pars triangularis) (BA45) of the brain. Since then, the approximate region he identified has become known as Broca's area, and the deficit in language production as Broca's aphasia, also called expressive aphasia. Broca's area is now typically defined in terms of the pars opercularis and pars triangularis of the inferior frontal gyrus, represented in Brodmann's cytoarchitectonic map as Brodmann area 44 and Brodmann area 45 of the dominant hemisphere.

Aphasia in the context of Alzheimer's disease

Alzheimer's disease (AD) is a neurodegenerative disease and is the most common form of dementia, accounting for around 60–70% of cases. The most common early symptom is difficulty in remembering recent events. As the disease advances, symptoms can include problems with language, disorientation (including easily getting lost), mood swings, loss of motivation, self-neglect, and behavioral issues. As a person's condition declines, they often withdraw from family and society. Gradually, bodily functions are lost, ultimately leading to death. Although the speed of progression can vary, the average life expectancy following diagnosis is three to twelve years.

The causes of Alzheimer's disease remain poorly understood. There are many environmental and genetic risk factors associated with its development. The strongest genetic risk factor is from an allele of apolipoprotein E. Other risk factors include a history of head injury, clinical depression, and high blood pressure. The progression of the disease is largely characterised by the accumulation of malformed protein deposits in the cerebral cortex, called amyloid plaques and neurofibrillary tangles. These misfolded protein aggregates interfere with normal cell function, and over time lead to irreversible degeneration of neurons and loss of synaptic connections in the brain. A probable diagnosis is based on the history of the illness and cognitive testing, with medical imaging and blood tests to rule out other possible causes. Initial symptoms are often mistaken for normal brain aging. Examination of brain tissue is needed for a definite diagnosis, but this can only take place after death.

Aphasia in the context of Pierre Paul Broca

Paul Pierre Broca (/ˈbrkə/, also UK: /ˈbrɒkə/, US: /ˈbrkɑː/, French: [pɔl bʁɔka]; 28 June 1824 – 9 July 1880) was a French physician, anatomist and anthropologist. He is best known for his research on Broca's area, a region of the frontal lobe that is named after him. Broca's area is involved with language. His work revealed that the brains of patients with aphasia contained lesions in a particular part of the cortex, in the left frontal region. This was the first anatomical proof of localization of brain function.

Broca's work contributed to the development of physical anthropology, advancing the science of anthropometry, and craniometry, in particular, the now-discredited practice of determining intelligence. He was engaged in comparative anatomy of primates and humans and proposed that Negroes were an intermediate form between apes and Europeans. He saw each racial group as its own species and believed racial mixing eventually led to sterility.

Aphasia in the context of Expressive aphasia

Expressive aphasia (also known as Broca's aphasia) is a type of aphasia characterized by partial loss of the ability to produce language (spoken, manual, or written), although comprehension generally remains intact. A person with expressive aphasia will exhibit effortful speech. Speech generally includes important content words but leaves out function words that have more grammatical significance than physical meaning, such as prepositions and articles. This is known as "telegraphic speech". The person's intended message may still be understood, but their sentence will not be grammatically correct. In very severe forms of expressive aphasia, a person may only speak using single word utterances. Typically, comprehension is mildly to moderately impaired in expressive aphasia due to difficulty understanding complex grammar. A better way to describe aphasia is fluent or non-fluent rather than "expressive" or "receptive" given the typical presence of both expressive and receptive language deficits in all subtypes of aphasia.

It is caused by acquired damage to the frontal regions of the brain, such as Broca's area. Expressive aphasia contrasts with receptive aphasia, in which patients are able to speak in grammatical sentences that lack semantic significance and generally also have trouble with comprehension. Expressive aphasia differs from dysarthria, which is typified by a patient's inability to properly move the muscles of the tongue and mouth to produce speech. Expressive aphasia also differs from apraxia of speech, which is a motor disorder characterized by an inability to create and sequence motor plans for conscious speech.

Aphasia in the context of Transient ischemic attack

A transient ischemic attack (TIA), commonly known as a mini-stroke, is a temporary (transient) stroke with noticeable symptoms that end within 24 hours. A TIA causes the same symptoms associated with a stroke, such as weakness or numbness on one side of the body, sudden dimming or loss of vision, difficulty speaking or understanding language or slurred speech.

All forms of stroke, including a TIA, result from a disruption in blood flow to the central nervous system. A TIA is caused by a temporary disruption in blood flow to the brain, or cerebral blood flow (CBF). The primary difference between a major stroke and a TIA's minor stroke is how much tissue death (infarction) can be detected afterwards through medical imaging. While a TIA must by definition be associated with symptoms, strokes can also be asymptomatic or silent. In a silent stroke, also known as a silent cerebral infarct (SCI), there is permanent infarction detectable on imaging, but there are no immediately observable symptoms. The same person can have major strokes, minor strokes, and silent strokes, in any order.

Encephalitis (US: /ɛnsɛfəlˈtɪs/, UK: /ɛnkɛfəlˈtɪs/) is inflammation of the brain. The severity can be variable with symptoms including reduction or alteration in consciousness, aphasia, headache, fever, confusion, a stiff neck, and vomiting. Complications may include seizures, hallucinations, trouble speaking, memory problems, and problems with hearing.

Causes of encephalitis include viruses such as herpes simplex virus and rabies virus as well as bacteria, fungi, or parasites. Other causes include autoimmune diseases and certain medications. In many cases the cause remains unknown. Risk factors include a weak immune system. Diagnosis is typically based on symptoms and supported by blood tests, medical imaging, and analysis of cerebrospinal fluid.

Aphasia in the context of Stutter

Stuttering, also known as stammering, is a speech disorder characterized externally by involuntary repetitions and prolongations of sounds, syllables, words, or phrases as well as involuntary silent pauses called blocks in which the person who stutters is unable to produce sounds. Almost 80 million people worldwide stutter, about 1% of the world's population, with a prevalence among males at least twice that of females. Persistent stuttering into adulthood often leads to outcomes detrimental to overall mental health, such as social isolation and suicidal thoughts.

Stuttering is not connected to the physical ability to produce phonemes (i.e. it is unrelated to the structure or function of the vocal cords). It is also unconnected to the structuring of thoughts into coherent sentences inside sufferers' brains, meaning that people with a stutter know precisely what they are trying to say (in contrast with alternative disorders like aphasia). Stuttering is purely a neurological disconnect between intent and outcome during the task of expressing each individual sound. While there are rarer neurogenic (e.g. acquired during physical insult) and psychogenic (e.g. acquired after adult-onset mental illness or trauma) variants, the typical etiology, development, and presentation is that of idiopathic stuttering in childhood that then becomes persistent into adulthood.

Walter Bruce Willis (born March 19, 1955) is a retired American actor.
